Abstract
Увод: Дишенова мишићна дистрофија (ДМД) је најчешћа наследна болест мишића са почетком у дечјем добу. Варијабилност клиничког тока ДМД делимично је објашњена варијантама у модификујућим генима. Циљеви: Одређивање учесталости алела варијанти гена SPP1, LTBP4 и CD40 и испитивање њихове појединачне и удружене повезаности са клиничком прогресијом ДМД у групи покретних и непокретних болесника. Материјал и методе: Генотипизоване су варијанте у SPP1 - rs28357094 и CD40 - rs1883832, као и хаплотип у LTBP4 гену - rs2303729, rs1131620, rs1051303, rs10880, тестом алелне дискриминације, код 95 болесника. Мера исхода била је време губитка хода, односно постигнућа на моторним скалама током једногодишњег периода праћења. Појединачни ефекат варијанти процењен је анализама преживљавања, а удружени хијерархијском кластер анализом. Резултати: Оболели који су користили кортикостероидну (КС) терапију изгубили су кретање годину дана касније (p=0,021). Учесталост ређих алела за анализиране варијанте била је у складу са учесталошћу за европску популацију. Модификујући ефекат. варијанти SPP1 и CD40 и хаплотипова LTBP4 није описан коришћењем log-rang теста и мултиваријантне Cox регресионе анализе. Кластер анализа је открила две подгрупе са статистичким трендом у разлици у годинама у време губитка хода. У групи покретних болесника мањи пад вредности на моторним скалама имали су носиоци заштитне варијанте у испитиваним генима. Закључци: Модификујући ефекат SPP1, LTBP4 и CD40 на клинички ток ДМД није описан иако је кохорта била упоредива по типовима мутација у DMD гену, фреквенцији варијанти одабраних гена и позитивном ефекту КС терапије са другим европским кохортама. Кластер анализа може да идентификује подгрупе које носе комбинацију генетских варијанти које модификују узраст у време губитка хода.
Introduction: Duchenne muscular dystrophy (DMD) is the most common inherited muscle disease with onset in childhood. Clinical course variability in DMD is partially explained by variants in modifier genes. Objectives: Determining the allele frequency of SPP1, LTBP4 and CD40 gene variants and assessing their individual and combined association with clinical progression of DMD in a group of ambulant and nonambulant patients. Material and methods: The variants in SPP1 - rs28357094 and CD40 - rs1883832, as well as the haplotype in the LTBP4 gene - rs2303729, rs1131620, rs1051303, rs10880, were genotyped by allelic discrimination test in 95 patients. The outcome measure was age at loss of ambulation (LoA), or achievement on the motor scales during the one year follow up period. The individual effect of the variants was assessed by survival analysis, and the combined effect by hierarchical cluster analysis. Results: Patients who used corticosteroids (CS) have LoA one year later (p=0.021). The frequency of rarer alleles of the analyzed variants was consistent with the frequency for the European population. The modifying effect of SPP1 and CD40 variants and LTBP4 haplotypes was not observed using log-rank test and multivariant cox regression analysis. Cluster analysis revealed two subgroups with a statistical trend in difference in age at LoA. In the group of ambulant patients, a smaller decrease in values on the motor scales had patients with protective variants in the studied genes. Conclusions: Modifying effect of SPP1, LTBP4 and CD40 on the clinical course of DMD was not replicated although the cohort was comparable in DMD mutation type distribution, allele frequencies of selected gene modifiers and positive effect of CS therapy, with other European cohorts. Cluster analysis may identify patient subgroups carrying a combination of the genetic variants that modify LoA.
