| dc.description.abstract | Bolničke infekcije (BI) predstavljaju globalni javnozdravstveni problem koji ugrožava zdravlje hospitalizovanih bolesnika i značajno opterećuje medicinsko osoblje i zdravstveni sistem. BI produžavaju lečenje, mogu dovesti do smrtnog ishoda, i povećavaju ukupne troškove lečenja. Intenzivna nega je deo sistema zdravstvene zaštite u kome se sreću najteži oblici bolničkih infekcija, sa uzročnicima koji su često rezistentni na većinu antibiotika. Njihova prevencija zahteva multidisciplinarni pristup kroz rigorozan i konstantan nadzor, ranu identifikaciju, adekvatno lečenje, i primenu pravovremenih mera u skladu sa lokalnim profilom infekcija i njihovim uzročnicima, kao i analizom isplativosti. Nadzor je posebno važan u nepredviđenim okolnostima kao što je KOVID-19 pandemija, tokom koje je došlo do promena u tipu infekcija i njihovim uzročnicima, zahtevajući i hitne izmene preporuka za prevenciju i lečenje. U ovoj studiji, analizirani su podaci konsekutivnih pacijenata hospitalizovanih u jedinici intenzivne nege Klinike za infektivne i tropske bolesti Univerzitetskog Kliničkog Centra Srbije u periodu od 5 godina (1.9.2016–1.9.2021.), u cilju utvrđivanja zastupljenosti bolničkih infekcija i procene njihovih karakteristika – tipova infekcija, uzročnika i osetljivosti uzročnika na antibiotike. Takođe, sprovedena je analiza trenda svih ispitivanih karakteristika bolničkih infekcija tokom celokupnog perioda praćenja, ali i u odnosu na pojavu KOVID-19 epidemije u Srbiji. Ispitivani su i faktori rizika za dobijanje bolničke infekcije tokom hospitalizacije u intenzivnoj nezi, uticaj bolničkih infekcija na mortalitet, kao i prediktori mortaliteta kod pacijenata sa bolničkim infekcijama. Kod pacijenata sa dijagnozom KOVID-19, ispitivan je uticaj bolničkih infekcija na ukupne troškove lečenja, podeljene po kategorijama: materijali, usluge, lekovi, laboratorjiska ispitivanja i bolnički dani. Ekonomskom analizom utvrđeni su dodatni troškovi bolničkih infekcija, ukupno i prema broju epizoda bolničkih infekcija tokom hospitalizacije. Tokom petogodišnjeg perioda praćenja, 980 pacijenata je uključeno u studiju. BI je imalo 278 pacijenata (28.4%), a od njih, trećina je imala više od jedne BI (97, 34.9%). | dc.description.abstract | Hospital-acquired infections (HAIs) are a global public health concern that put patients at risk and cause a significant on the healthcare staff and hospital systems. They prolong hospitalization, can lead to complications, including death, and increase treatment costs. The most severe forms of HAIs are found in intensive care units, with causative agents frequently resistant to most, if not all, antibiotics used for treatment. Their prevention necessitates a multidisciplinary approach through rigorous and constant surveillance, early detection, appropriate treatment, and adequate implementation of measures based on the local profile of infections and causative agents, but also economic evaluations. Surveillance is particularly important in times like the COVID-19 pandemic, during which we saw changes in the profile of HAIs, calling for immediate modifications of infection prevention and treatment guidelines. In this study, data of patients consecutively admitted to the intensive care unit of the Teaching Hospital for Infectious and Hospital Diseases, University Clinical Center of Serbia in a 5-year period (September 1, 2016 – September 1, 2021) were analyzed to establish rates of HAIs and their characteristics – types, causative agents and antimicrobial susceptibility testing (AST). Trend analysis was performed for all of the examined characteristics throughout the entire follow-up period, as well as their comparison before and during the COVID-19 pandemic in Serbia. Risk factors for acquiring HAIs in the intensive care unit, their impact on mortality, and predictors of mortality in patients who acquired HAIs were evaluated. In patients with a diagnosis of COVID-19, the impact of HAIs on total treatment costs were analyzed, divided into several categories: materials, services, drugs, laboratory testing, and occupancy costs. Through an economic analysis, incremental costs of HAIs were determined, in total and in relation with the number of acquired infections during hospitalization.
