    Amyand's hernia: Cause of inguinal swelling

    Amyand's hernia is a condition where the vermiform appendix is contained within an inguinal hernia sac. An inguinal hernia with an appendix in the hernia sac is extremely rare and almost exclusively occurs on the right side. The mechanism of developing appendicitis with hernia is most likely the compression of the appendix at the level of the neck of the sac or due to impaired vascular supply leading to infection. Despite technological advances in radiology modalities, the preoperative diagnosis of and hernia is challenging: the role of abdominal ultrasound and computed tomography is very important and is essential in planning surgical management and approaches. In our case, we made a rare preoperative diagnosis of Amyand's hernia using ultrasound showing a typical appearance only without wall hyperemia probably due to vascular compression of the inguinal ring, which was confirmed by CT. CT is more sensitive to the detection of radiological signs of appendiceal inflammation and perforation in the hernia sac and is most often used in the case of an inconclusive ultrasound findings. Due to the prompt radiological diagnosis of acute appendicitis in the hernia sac provided by ultrasound, our patient underwent laparoscopic appendectomy. © 2024 Australasian Sonographers Association.

    Diagnostics and treatment of liver injuries in polytrauma

    Trauma is one of the most serious and most expensive health problems and most common cause of death in youth. In last decade, non operative treatment is the standard treatment strategy in blunt liver injuries in haemodynamic stable patients. Methods: restrospective study included patients with liver injury admitted between May 2005 and May 2012. in the Center for Emergency Surgery of the Clinical Center of Serbia. Total number of patients was 224. Results: Out of 224 patients with liver injuries, 183 had blunt injury, and only 41 had penetrant injury. Isolated injury was found in 25.9% of patients, and 74.1% had multiple injuries. Average Injury Severity Score (ISS) was 22.48 (SD=12.16). Patients underwent intraoperative classification according to Moore. In 2% of patients who were haemodynimically instable we used "damage control surgery". Out of 224 patients, 89,7% were successfully treated, 2,7% died as "mors in tabula", or within first 24 hours, and 7.6% died during hospitalization. Conclusion: Modern diagnostic procedures provided a much more precise verification of liver injuries associated with polytrauma and reduced mortality. Higher percentage of patients with nonoperative treatment was among patients with ISS lower than 10, and among those with injuries grades I and II.

    Knowledge and awareness of tuberculosis among medical and dental students in belgrade, Serbia

    Aim: Tuberculosis (TB) continues to be an important health problem in Serbia, as a country with numerous socioeconomic problems. Health care workers, especially medical and dental students could be key persons to engage in prevention of TB. The aim of our study was to compare the knowledge level and awareness of TB among medical and dental students. Methods: Cross-sectional study was conducted at the School of Medicine and the School of Dentistry, University of Belgrade, Serbia. A sample of 350 students was selected by stratified sampling.Data about knowledge and awareness of TB was collected through the anonymous (self-administered) questionnaire. Results: 300 questionnaires were returned; response rate was 85.7%. Medical students gave significantlyhigher correct responses about modes of transmission of TB (p<0.001). Significantly higher proportion of medical students (p=0.003) knew that age above 65 years is risk period of life for getting TB, but only 6.5% of them noted that the age of puberty is the risk period too. High proportion of both groups agreed that alcoholism and AIDS are the conditions, which can increase risk for getting TB. Medical students have better knowledge about therapy of TB, but there were no differences concerning questions about diagnosis. Conclusion: In spite of valid basic awareness of TB, there were some gaps in the knowledge, more frequently among dental then medical students. This study suggests that students need more training and more practice.
