Publication A simple index for nonalcoholic steatohepatitis—HUFA—based on routinely performed blood tests(2019) ;Culafic, Milica (55881915300) ;Kovacevic, Sandra Vezmar (57204567668) ;Dopsaj, Violeta (6507795892) ;Stulic, Milos (55895099100) ;Vlaisavljevic, Zeljko (56461417200) ;Miljkovic, Branislava (6602266729)Culafic, Djordje (6603664463)Background and objectives: Data suggests that nearly 30% of the general population have steatosis and up to 5% of this population develops nonalcoholic steatohepatitis (NASH). Liver biopsy is still considered to be the gold standard for the diagnosis of NASH. Great effort is being made toward the identification of sensitive diagnostic tests that do not involve invasive procedures to address a common concern in patients with the nonalcoholic fatty liver disease—whether they have NASH or simple steatosis. We aimed to investigate the independent predictors and develop a non-invasive, easy-to-perform, low-cost set of parameters that may be used in clinical practice to differentiate simple steatosis from NASH. Methods: A cross-sectional study of nonalcoholic fatty liver disease (NAFLD) patients divided into two groups: group I—simple steatosis (SS) and group II—biopsy-proven NASH. Strict inclusion criteria and stepwise analysis allowed the evaluation of a vast number of measured/estimated parameters. Results: One hundred and eleven patients were included—82 with simple steatosis and 29 with biopsy-proven NASH. The probability of NASH was the highest when homeostatic model assessment of insulin resistance (HOMA-IR) was above 2.5, uric acid above 380 µmol/L, ferritin above 100 µg/L and ALT above 45 U/L. An acronym of using first letters was created and named the HUFA index. This combined model resulted in an area under the receiver operator characteristic curve (AUROC) of 0.94, provided sensitivity, specificity, positive predictive value and a negative predictive value for NASH of 70.3%, 95.1%, 83.1% and 90.0%, respectively. Conclusion: We suggest a simple non-invasive predictive index HUFA that encompasses four easily available parameters (HOMA-IR, uric acid, ferritin and ALT) to identify patients with NASH, which may reduce the need for a liver biopsy on a routine basis in patients with NAFLD. © 2019 by the authors. Publication Attitudes of Nurses Toward Organ Donation in Serbia(2020) ;Vlaisavljevic, Zeljko (56461417200) ;Jankovic, Slobodan (7101906319) ;Maksimovic, Natasa (12772951900) ;Culafic, Milica (55881915300) ;Stulic, Milos (55895099100) ;Milovanovic, Tamara (55695651200)Oluic, Branislav (57201078229)Objective: Nurses represent the key persons in the process of organ donation, acting as intermediaries between the patient and the family of a potential donor. The aim of this study was to analyze the factors influencing the attitudes of nurses toward bequeathing and organ and tissue donation. Methods: The research was designed as a cross-sectional study from November 2013 to November 2014 and included a sample of 264 nurses employed in the health system of Serbia. Data were collected using a specific questionnaire of 18 questions referring to the information on sociodemographic characteristics and the knowledge of organ donation. Results: The investigation enrolled 264 participants, of which 78% were women. The majority of study subjects had secondary medical education (64.4%), lived in an urban area (82.6%), were married (56.4%), and were predominantly Orthodox (87.1%) with their work experience between 5 and 10 years (29.9%) and 10 to 20 years (29.5%). The average knowledge score concerning organ donation was 29.03 (SD, 2.33; range, 23-34). There was a statistically significant difference in the scores between 2 groups formed according the length of service (P < .001) and the level of education (P = .019), which showed the strongest influence on nurses’ attitudes toward donation. Conclusion: This investigation showed that nurses did not express strong positive attitude toward this issue. Further education of nurses in the Serbian health care system focusing on transplantation and bequeathing of organs and tissues are warranted. © 2020 Elsevier Inc. - Some of the metrics are blocked by yourconsent settings
Publication Bacteremia in patients with liver cirrhosis in the era of increasing antimicrobial resistance: single-center epidemiology(2021) ;Milovanovic, Tamara (55695651200) ;Pantic, Ivana (57223613349) ;Velickovic, Jelena (29567657500) ;Oluic, Branislav (57201078229) ;Vlaisavljevic, Zeljko (56461417200) ;Dragasevic, Sanja (56505490700) ;Lalosevic, Milica Stojkovic (57218133245)Dumic, Igor (57200701725)Introduction: Liver cirrhosis is commonly associated with bacterial infections, which contribute to unfavorable outcome. This study aimed to investigate the epidemiology of bacteremia and patterns of antibiotic resistance in patients with cirrhosis, factors associated with multidrug-resistant infection, and predictors of mortality. Methodology: This retrospective single-center study included patients with cirrhosis treated between January 2016 and December 2018. Data were collected from the patients’ medical records. The severity of liver disease was determined using the Child–Pugh, Model for End-Stage Liver Disease-Na, Chronic Liver Failure-Consortium Acute-on-Chronic Liver Failure, and Chronic Liver Failure-Consortium Acute Decompensation scores. Results: A total of 85 patients with cirrhosis and bacteremia were included (male: 82.4%, mean age 60.3 ± 9.4 years). The etiology of cirrhosis was mainly alcoholism (87.1%). After 30 days, lethal outcome occurred in 44.7% of the patients. The most commonly isolated pathogens were Enterococcus spp. (31.8%), methicillin-sensitive Staphylococcus aureus (15.3%), and Escherichia coli (14.1%), while 37.3% of all isolated microorganisms were multi-drug resistant. Multi-drug resistant infection [odds ratio (OR): 6.198, 95% confidence interval (CI): 2.326–17.540, p = 0.006] and neutrophil-to-lymphocyte ratio (OR = 1.181, 95% CI = 1.043–1.337, p = 0.009) are independent predictors of mortality. The aforementioned scores, which represent the extent of hepatic insufficiency, are significantly higher in patients with multi-drug resistant isolates, while multi-drug resistant bacteremia was more common in patients with more advanced liver disease. Conclusions: Multi-drug resistant bacteremia is more common in patients in whom liver disease is more severe and is a major independent predictor of mortality. Publication Cirrhosis Due to Alcohol-Related Liver Disease Hospitalizations in Belgrade, Serbia: A 10-Year Retrospective(2022) ;Milovanovic, Tamara (55695651200) ;Lugonja, Sofija (57219557462) ;Pantic, Ivana (57223613349) ;Miltenovic, Svetlana (57915216800) ;Vlaisavljevic, Zeljko (56461417200)Mardani, Abbas (57216255421)Background: Cirrhosis due to alcohol-related liver disease (ALD cirrhosis) is a significant burden to health systems worldwide. We aimed to determine the trends in hospitalization frequency due to ALD cirrhosis and to analyze their characteristics. Methods: This cross-sectional study used data from the Institute of Public Health of Belgrade database, and included all hospitalization reports which contained code K70.3 (Cirrhosis hepatis alcoholica) as the primary diagnosis, including re-hospitalizations, on the territory of Belgrade, between January 2009 and December 2018. Results: A total of 4644 patients with ALD cirrhosis were hospitalized (male: 4154, 89.45%), with a mean age of 58.83±10.02 years. During the 2009-2018 decade, no difference in the number of ALD cirrhosis hospitalizations in subsequent years was observed. Men more commonly developed esophageal and gastric varices with bleeding compared to women (P=0.037), while women developed acute-on-chronic liver failure (ACLF) al-most two-times more often compared to men (P<0.001). Patients with hepatocellular carcinoma were significantly older (P<0.001), while those who developed ascites and splenomegaly were significantly younger compared to those who did not (P<0.001 and P=0.04, respectively). Altogether, complications of portal hypertension were registered and reported with very low frequency, and therefore do not represent actual frequencies of these conditions. The median duration of hospital stay was 9 days (range 0-243). Patients in whom lethal outcome occurred during the hospitalization were significantly older, and more commonly developed chronic renal failure. Conclusion: These data offer an important insight into the ALD cirrhosis-related hospitalizations while draw-ing attention to inadequate coding as an important public health issue at the same time. © 2022 Milovanovic et al. Publication Intrahepatic cholestasis of pregnancy: A case study of the rare onset in the first trimester(2019) ;Stulic, Milos (55895099100) ;Culafic, Djordje (6603664463) ;Boricic, Ivan (6603959716) ;Lalosevic, Milica Stojkovic (57218133245) ;Pejic, Nina (57210712517) ;Jankovic, Goran (7005387171) ;Milovanovic, Tamara (55695651200) ;Culafic-Vojinovic, Violeta (16686529100) ;Vlaisavljevic, Zeljko (56461417200)Culafic, Milica (55881915300)Intrahepatic cholestasis of pregnancy (ICP) is a gestation-specific liver disorder, defined most often as the onset of pruritus, usually from the third trimester of pregnancy, associated with abnormal liver test results and/or increased total serum bile acids and spontaneous relief after delivery. The 21-year-old patient was admitted to our ward in the 11th week of pregnancy due to raised liver enzymes. The first onset of pruritus and jaundice appeared a month before hospitalization. Immunology tests and Toxoplasma gondii were negative. We excluded viral etiology, while alpha-1-antitrypsin, serum and urine copper levels, and thyroid hormones were within the reference values. The patient denied she had taken any medicines and herbal preparations before and during pregnancy. Total bile acids in the serum were significantly elevated (242 µmol/L). The abdominal ultrasound revealed a regular finding. Liver biopsy suggested a cholestatic liver disorder. After a presentation of all risks, the patient decided to stop the pregnancy. After a month, the hepatogram was within the reference values. Very rarely an ICP can occur in early pregnancy (first trimester), which calls for close monitoring. The risk of serious adverse fetal outcomes and spontaneous preterm delivery is proportional with increased levels of maternal serum bile acid. © 2019 by the authors. Publication The Quality of Life After Liver Transplantation—The First Experience From Serbia(2022) ;Oluić, Branislav (57201078229) ;Jadrijevic, Stipislav (6507816941) ;Pantic, Ivana (57223613349) ;Dragasevic, Sanja (56505490700) ;Popovic, Dusan (37028828200) ;Lalosevic, Milica Stojkovic (57218133245) ;Vlaisavljevic, Zeljko (56461417200) ;Abdi, Alireza (56845014200)Milovanovic, Tamara (55695651200)Background: Liver transplantation (LT) is the most effective treatment for patients with end-stage liver disease, which dramatically effects patient’s quality of life (QoL). The aim of this study was to evaluate the impact of socio-demographic and clinical factors on different QoL domains of patients who underwent orthotopic LT. Methods: A cross-sectional study included a total of 43 patients who underwent a LT from 2013 to 2018. Socio-demographic and clinical data were recorded in each patient. The QoL was estimated using two validated questionnaires: 36-Item Short Form Health Survey (SF-36) and Chronic Liver Disease Questionnaire (CLDQ). Results: Male patients obtained significantly higher scores than women, in the domains of general health perception (83.2 ± 16.3 vs. 71.0 ± 18.4; t = 2.229, p =.031) and physical component summary (69.0 ± 7.2 vs. 62.0 ± 11.4; t = 2.451, p =.019). There were no significant differences in other domains of SF-36 and CLDQ. Etiology of the underlying liver disease and the presence of post-transplant complications showed no effect on score values of SF-36 and CDLQ domains (p <.05). Time from LT showed negative medium correlation with role limitations due to physical health problems (S = −0.417, p =.005), while no other significant correlations were noted in other items of SF-36 and CLDQ. Conclusions: Men had higher scores in the domain of general health perception and physical component summary following LT than women. With the increase in time from LT, patients experience a decrease in limitations due to physical health problems. The audit and improvement of QoL is an essential part of the individualized long-term health-care approach to LT patients. © 2022 Springer Publishing Company. - Some of the metrics are blocked by yourconsent settings
