    Correlation between conjunctival scraping cytology and other clinical dry eye metrics in determination of dry eye related inflammation

    Background/Aim. New and improved definition of dry eye disease (DED) emphasized that hiperosmolarity and inflammation with initial tear film instability play etiological role. The aim of this study was to explore relation of some commonly used clinical tests to dry eye disease (DED) related inflammation measured by conjunctival scraping cytology. Methods. We examined 100 subjects, 80 of them having DED. We performed Schirmer without anesthesia (Schirmer I), Fluorescein Tear Break Up Time (FTBUT), Rose Bengal (RB), Lid Parallel Conjunctival Folds (LIPCOF), Tear Meniscus Height (TMH) and Tear Ferning (TF) and compared the values to scraping scores of tarsal conjunctiva. Results. FTBUT had the best sensitivity (93.6%). The highest specificity was found with RB (93.2%), but it was also high with Schirmer I, TF and FTBUT (respectively 89.8%, 84.5%, 78.0%). RB and FTBUT had the highest correlation with conjunctival scraping score (r = 0.707, p < 0.001; r = -0.507, p < 0.001). Conclusion. In our study, FTBUT, though often used in many combinations of the DED tests, showed a remarkably high sensitivity and specificity on its own, as well as good correlation with DED related inflammation detected with conjunctival scraping cytology. © 2019 Inst.     Diagnostic value of three simple and rapid dry eye tests – lid parallel conjunctival folds, tear meniscus height, and tear ferning

    Introduction/Objective The objective of this paper was to assess the diagnostic value of three simple dry eye (DE) tests: lid parallel conjunctival folds (LIPCOF), tear meniscus height (TMH), and tear ferning (TF). Methods LIPCOF, TMH, and TF diagnostic DE tests were performed in 100 patients. Eighty of them were referred to us by rheumatologists and general practitioners either during evaluation for Sjögren’s syn-drome, or because of DE symptoms. The control group was composed of 20 patients, with no DE-related symptoms. Ocular Surface Disease Index questionnaire was used for DE symptoms’ evaluation. Results of LIPCOF, TMH, and TF tests were compared with results of the Copenhagen criteria DE tests i.e., tear fluorescein breakup time, Schirmer I and Rose Bengal tests. Ability of the tests to recognize DE in various grades according to Dry Eye Work Shop (DEWS) report score system was assessed. Results Compared to the Copenhagen criteria, sensitivity of LIPCOF and TMH was high (92.8% and 83.5%, respectively), while specificity was low (34.4% and 49.2%, respectively). TF had low sensitivity (59.1%) but high specificity (82.7%). Mean values of both LIPCOF and TMH differed significantly (F = 7.222, p < 0.001 and F = 11.802, p < 0.001, respectively) between the control group and all DEWS grades, but not among different grades of DE. Conclusion TMH and LIPCOF diagnostic tests showed high sensitivity, which makes them excellent screening DE tests. Low sensitivity of TF suggests that it is not truly a good screening test on its own, but its high specificity is of definite value. © 2021, Serbia Medical Society. All rights reserved.

    Persistent submacular fluid diagnosed with Optical Coherence Tomography after successful scleral buckle surgery for macula-off retinal detachment

    The objective of our study was to analyze the presence of persistent submacular fluid after apparently successful scleral buckle surgery for macula-off retinal detachment, using Optical Coherence Tomography (OCT), and to correlate these findings to postoperative visual acuity. Prospective study of consecutive patients hospitalized for macula-off retinal detachment, between February 2008 and April 2010, was carried out at the single referral centre for vitreoretinal surgery in Serbia. Outcomes were evaluated 1, 3, 6 and 12 months postoperatively, using OCT and best corrected visual acuity (BCVA). All 48 patients with macula-off retinal detachment had undergone clinically successful scleral buckle surgery, 3 to 30 days (mean 14.2 ± 6.9) after the onset of symptoms. Reattachment was achieved and one month later, patients were divided in two groups, according to the presence of submacular fluid assessed by OCT: group A with detectable residual submacular fluid (40%) and group B without (60%). The mean BCVA one month postoperatively was significantly lower (p<0.01) in the group A (0.11 ± 0.03) opposed to the group B (0.51 ± 0.11). However, after a 12-month-follow-up, BCVA was similar in both groups (0.52 ± 0.17 and 0.54 ± 0.15 respectively, p=0.541), due to the restitution of normal macular architecture confirmed by OCT. The prolonged recovery of visual acuity in patients with macula-off retinal detachment who had undergone successful scleral buckle surgery could be explained with presence of submacular fluid, diagnosed by OCT. However, final visual outcome was favourable, due to the gradual resorption of submacular fluid in a 12-month-period. © 2012 Association of Basic Medical Sciences of FBIH. All rights reserved.
