Introduction. Castleman disease is a rare disease of the unknown etiology, occuring in two clinical forms: unicentric or multicentric. It is characterized by the hyperplasia of lymph glands. In literature the four pathohistological forms were described: hyaline vascular type, plasma cell type, mixed type and a recently recognized plasmablastic type. The most frequent changes are localized in the mediastinum, while the abdominal localization is with significantly rare occurrence, and that was the motive for presentation of this case. Case report. In a 41-year old male magnetic resonance (MR) enterography showed a change in the ileocecal area without the presence of subjective symptoms of digestive tract and without loss of body mass. Due to the suspicion of stromal tumor, surgical intervention was indicated. Pathohistological findings showed Castleman lymphadenopathia reactiva mesenterii (plasma cell type) which was in the unicentric form. There were present only anaemia and the increased value of sedimentation from the laboratory analyses. Conclusion. Abdominal localization of unicentric plasma cell form occurs rarely and the surgical method of treatment presents the golden standard as it was shown in the presented case. © 2017, Institut za Vojnomedicinske Naucne Informacije/Documentaciju. Introduction. Autoimmune pancreatitis is a disease associated with autoimmune mechanisms, clinically manifested mostly as obstructive icterus with or with no entire or partial enlargementof the pancreas, histological lymphoplas-mocytic infiltration, fibrosis or granulocytic epithelial lesions with a favourable therapeutic response to the application of corticosteroids. Type 1 autoimmune pancreatitis is a systemic disease befalling the group of IgG4-related diseases in contrast to type 2 which is specific for pancreas disease. Case report. We presented two cases. The first one was a 64-year-old male patient with autoimmune pancreatitis complaining of abdominal pain, weight loss, weakness and exhaustion. Clinical examination showed a rare IgG4 autoimmune pancreatitis. The second one was a 37-year-old male patient complaining of abdominal pain with diarrhea. The diagnosis made revealed the presence of type 2 autoimmune pancreatitis. Following the diagnosis, immunosuppressive therapy was administered to both patients leading to the improvement of their general condition. Conclusion. Autoimmune pancreatitis is a rare disease, sometimes not easy to differ from pancreatic tumor or bile duct tumor with poor prognosis. Thus, early recognition of the disease is very important, since adequate treatment significantly increases the course and the outcomes of the disease. © 2017, Institut za Vojnomedicinske Naucne Informacije/Documentaciju. Introduction. Erdheim-Chester disease (ECD) is a rare non-Langerhans-cell histiocytosis. About 500 cases are published so far. It is multisystemic disease characterised by bilateral symmetric long bones sclerosis. Main histopathological finding is accumulation of big foamy histiocytes, immunohistochemically positive to CD68, and negative to S-100 and CD1a. There are no guidelines that reliably identify population that requires therapy, but symptomatic ECD, organ failure and central nervous system involvement require treatment. Case report. We described a patient with a multisystemic form of ECD affecting long bones, the hypophysis, abdomen, and the peripheral nerves. Five years after initial symptoms ECD was suspected. Prednison was initiated, 60 mg once a day. After obtaining the diagnosis of ECD, interferon alpha 2A was introduced, but soon after stopped due to severe side effects. Considering that histiocytes were positive to platelet derived growth factor receptor alpha (PDGFR alpha) imatinib mesylate was started, but after two months stopped due to no clinical and radiological improvement. The disease was worsening and the patient died. Conclusion. We described the patient with intraperitoneal form of ECD, without cardiac and pulmonary involvement. There are several important issues: the diagnosis of ECD could be difficult to make, three treatment regimens were included and the patient died nine years after the initial symptoms due to indolent course of the disease and unsuccessful treatment. © 2017, Institut za Vojnomedicinske Naucne Informacije/Documentaciju. Background/Aim. Arterial stimulation with calcium and venous sampling (ASVS) enables us to reach the goal of avoiding that any patient with insulinoma undergoes a blind surgical exploration. Since ASVS is both a functional and morphological localization procedure, its sensitivity is not influenced by factors that are causing the insensitivity of usual anatomical and morphological procedures. Based on our own experience in preoperative localization of insulinoma, we indented to show why we believe that ASVS should be performed to all patients regardless of data collected from other preoperative localization methods. Methods. We have analyzed the accuracy of preoperative localization methods retrospectively. First anatomical and morphological procedures like transabdominal ultrasound (US), endoscopic ultrasound (EUS), computed tomography (CT) and magnetic resonance imaging (MRI) were done. Then we analyzed the data collected during a functional procedure which, at the same time, allows regionalization (ASVS). To estimate the accuracy, the results of every single method were correlated with the operative findings in all sixteen cases. Results. Prior to ASVS, fourteen patients underwent US, fifteen had CT, MRI was performed in eight patients and EUS in thirteen. Using only one of these methods enabled identification of tumors in five patients, using two methods in six patients while three and four in one patient each. For three patients, none of these methods was successful. ASVS revealed that all seen tumors were functional except three of the six visualized with two methods (US and EUS). In two of these three cases, US and EUS localized the tumors in pancreatic tail/body while ASVS accurately identified the tumors in pancreatic head. For these patients US and EUS showed false positive results. In the third of these patients EUS showed the tumor localized in pancreatic head, while US and ASVS accurately pointed to tail. This, too, was a false positive result of EUS. ASVS successfully provided regionalization data in three patients where other visualization methods failed. Operative and later histological findings confirmed the accuracy of ASVS in all sixteen patients including two patients that previously underwent distal pancreatectomy based on false positive EUS findings. Conclusion. Two patients, with accurate insulinoma regionalization in pancreatic head, obtained with ASVS, previously underwent unsuccessful distal pancreatectomy based on the false positive EUS findings. The same goes to three other patients with the false positive results obtained with other anatomical and morphological findings, as well as those three patients that had no preoperative visualization with other methods prior to ASVS. Therefore we suggest ASVS performing in each suspected insulinoma patient before the surgery, regardless of the data collected using other methods. This would enable us to test functional characteristics of visualized findings and to regionalize part of pancreas with uncontrolled insulin secretion when no suspicious changes were found. © 2018, Inst. Background/Aim. Gastric ulcer may be benign or malignant. In terms of therapy and patient’s prognosis early detection of malignancy is very important. The aim of this study was to assess the usefulness of endoscopic ultrasound (EUS) in differentiation between benign and malignant gastric ulcer. Methods. A prospective study included 20 consecutive adult patients with malignant gastric ulceration and 20 consecutive adult patients with benign gastric ulceration. All the patients underwent EUS. A total of 6 parameters were analyzed: ulcer width, ulcer depth, the thickness of the gastric wall along the edge of ulceration (T0), the thickness of the gastric wall 2 cm from the edge of ulceration (T2), loss of layering structure of the gastric wall, and the presence of regional lymph nodes. EUS criteria for malignancy and a point-score of malignancy were defined. The critical value of total point-score was also calculated showing the best reliability parameters. Results. There are 4 criteria for malignancy of gastric ulceration: T0 > 10 mm, T2 > 5 mm, EUS visualization of at least one lymph node, loss of layering structure of the gastric wall. Furthermore, T2 > 5 mm was the only EUS independent predictor of ulcer malignancy. The total point score of ≥ 4 was the cut-off point-score value which gave the best reliability parameters in the assessment of malignant ulcers: sensitivity of 70%, specificity of 95%, positive predictive value of 93.3%, negative predictive value of 76% and accuracy of 82.5%. Conclusion. According to the results obtained in this study, we can conclude that EUS is usefull in differentiation between benign and malignant gastric ulcer. © 2016, Institut za Vojnomedicinske Naucne Informacije/Documentaciju.
