Publication Amoxicillin-clavulanate prescribed in a patient with known penicillin allergy(2020) ;Anđelić, Slađana (35791554900) ;Savić, Slobodan (57219558702)Kozić, Duško (6602538657)Introduction Prescription of penicillin requires extra caution in order to avoid its administration in a person allergic to this antibiotic. We present a case of a patient allergic to penicillin, to whom a doctor prescribed this medicine by mistake. Case outline An 18-year-old female patient turned to an otolaryngologist because of a sore throat, dif-ficulty in breathing, and light clogging in the left ear during several previous days. The patient tolerated oral intake of only liquid foods. She reported frequent attacks of tonsillopharyngitis, and an allergy to penicillin. Tonsillopharyngitis was established by a physical examination. The doctor prescribed oral therapy, including a penicillin-based antibiotic Augmentin® (amoxicillin + clavulanate potassium) 1000 mg 2 × 1 tablet for seven days. The pharmacist in the local pharmacy knew the patient and was aware of the fact that the girl was allergic to penicillin, so the patient did not take the prescribed penicillin-based remedy. In this way, an extremely serious professional medical error did not obtain essential features of a criminal act according to the Serbian Criminal Code. Conclusion When prescribing antibiotics, it is necessary for the physician to be extremely careful not to prescribe a medicine for which there is a cave warning in medical documentation, as this error can become grounds for legal prosecution against the doctor, as well as for professional sanctioning. © 2020, Serbia Medical Society. The magnetic resonance imaging (MRI) was found to be insensitive diagnostic modality in detecting the abnormalities in patients with vestibular neuritis. Case report. A 32-year-old man was admitted to hospital with clinical signs of acute vestibular neuritis. The conventional MRI was inconclusive, including 3 mm slice-thickness postcontrast study, while the postcontrast high resolution study with 1 mm slice-thikness, detected bilateral enhancement of the vestibulocochlear nerve’s vestibular branch, consistent with inflammation. Conclusion. High-resolution 1 mm or sub-milimeter slices should be perfomed to evaluate patients with vestibular neuritis in order to increase the MRI sensitivity and improve correlation with clinical findings. © 2019, Inst. Publication White Matter Changes in Two Leber's Hereditary Optic Neuropathy Pedigrees: 12-Year Follow-Up(2016) ;Jančić, Jasna (35423853400) ;Dejanović, Ivana (55074744000) ;Radovanović, Saša (6604015284) ;Ostojić, Jelena (12797904900) ;Kozić, Duško (6602538657) ;Durić-Jovičić, Milica (26633912900) ;Samardžić, Janko (23987984500) ;Ćetković, Mila (9232864300)Kostić, Vladimir (57189017751)We are presenting two Leber's hereditary optic neuropathy (LHON) pedigrees with abnormal magnetic resonance imaging (MRI) and proton magnetic resonance spectroscopy (H-MRS) findings but without neurological manifestation associated with LHON. The study included 14 LHON patients and 41 asymptomatic family members from 12 genealogically unrelated families. MRI showed white matter involvement and H-MRS exhibited metabolic anomalies within 12 LHON families. Main outcome measures were abnormal MRI and H-MRS findings in two pedigrees. MRI of the proband of the first pedigree showed a single demyelinating lesion in the right cerebellar hemisphere, while the proband of the second family displayed multiple supratentorial and infratentorial lesions, compatible with the demyelinating process, and both the absolute choline (Cho) concentration and Cho/creatinine ratio were increased. MRI and H-MRS profiles of both affected and unaffected mitochondrial DNA mutation carriers suggest more widespread central nervous involvement in LHON. Although even after 12 years our patients did not develop neurological symptoms, MRI could still be used to detect possible changes during the disease progression. © 2015 S. Karger AG.
